Registration of outpatient care services

Anyone who provides or offers reserved activities in care in return for payment must notify the responsible health authority immediately.

Course description

In the city of Munich, outpatient care services must be reported immediately to the responsible health authority (in the city of Munich: Health Department, Department of Hygiene and Environmental Medicine), stating the name and address and, if applicable, the name and address of the facility: Health Department, Department of Hygiene and Environmental Medicine).

Required documents

  • Registration form (The registration form is created automatically when you use our online registration form. If you do not use the online form, please request the registration form from us. You will find the contact details at the bottom of this page)
  • Valid identification document
  • Licence certificate (geriatric nurse, nursing specialist, etc.) or "Description of professional training" form (see Forms / Downloads) if you do not have a licence certificate yourself
  • Details of the nursing service management with submission of the licence certificate
  • Confirmation of inspection of employee documents (see forms / downloads)

Fees and charges

Registration is free of charge and registration confirmations will only be issued on request. A fee of 18 euros will be charged for this.
