Corona vaccination certificate
If you have been vaccinated at the Munich Vaccination Centre or one of its branch offices, you can apply for replacement vaccination documents using an online form.
Description
Citizens who have been vaccinated at the Munich Vaccination Centre or one of its branch offices can apply for the following documents via the online form for a fee:
- Confirmation that you have been vaccinated against coronavirus (if you have lost the original)
- Copy of your corona vaccination file (medical history form, declaration of consent)
The documents will be sent to you in paper form by post.
Prerequisites
The relevant documents can only be sent if you have been vaccinated at the vaccination centre of the City of Munich (Messe München-Riem, Gasteig, Rathaus). If you require a certificate for a vaccination carried out by a registered doctor or pharmacy, please contact them.

- Identification document (identity card, passport)
- signed power of attorney, if applicable, if the documents are requested for a third party
Questions & Answers
With your application via the online form, you can only receive confirmation of completed COVID-19 vaccinations. Other vaccination confirmations, such as measles vaccinations, cannot be requested here.
The Health Department can only issue certificates or documents if the corona vaccination was carried out at the vaccination centre of the City of Munich (Messe München-Riem, Gasteig, Rathaus). If you were not vaccinated at the vaccination centre and still need vaccination documents, please contact the place where you were vaccinated.
Yes, but you must send a signed authorisation from the vaccinated person for whom you are requesting the documents.
You can also apply for confirmation for a minor child for whom you have custody.
No, you can apply for the required documents online. The documents will be sent to you by post and you do not need to attend an appointment.
