Promotion of care transition

As a full inpatient care facility, you can receive a personnel cost subsidy for a care transfer in accordance with the guidelines.

Description

Note On 29 November 2023, the Munich City Council decided to end the voluntary "Care Transition" programme , which has been successfully implemented since 1999, on 31 December 2024 (point 2.4 of the resolution). The funds released from this programme will be reallocated to finance school social work in vocational schools for nursing and to increase the voluntary services of the "In-Home Day Care" programme.

A celebration to mark the 25th anniversary of the funding programme will take place on 8 October 2024. Written invitations will be sent to the care referrals, facility and care service managers of the fully inpatient care facilities in Munich, representatives of the providers, associations and the city council as well as supporters of the funding programme.

The task of care transition
The care transition team, a certified care professional, supports and accompanies people when they move into a fully inpatient care facility. They perform the following tasks in particular:

- Advising and informing
- Organising and coordinating the move into the care facility
- Support during the familiarisation period
- Handing over information to the care team

Care transition concept
The "Care transition" concept from 2018 was developed in workshops together with the care transition team and agreed with the management of the Munich care home providers.

Prerequisites

All full inpatient care facilities that provide their services within the area of the City of Munich and have more than 30 full inpatient care places in accordance with the Social Code XI - Social Care Insurance (SGB XI) are eligible.
The guidelines for a care transfer set out all the requirements that must be met in order to apply for a personnel cost subsidy.
Applications (complete and in writing) can only be submitted using the application forms and documents provided (application for the calendar year, annual payroll account and report form for the previous year) by 31 March of the application year at the latest (date of receipt by the City of Munich).
Applications in electronic form (e.g. by e-mail) are not permitted. Applications (application and report form) that are not submitted on time will not be considered for funding.

As a fully inpatient care facility, you can receive funding for supervision, further education and training.
