Promotion of care transition

The care transition supports people when they move into a fully inpatient care facility.

Description

The task of care transition

The care transition, a registered nursing professional, supports and accompanies people when they move into a full inpatient care facility. They perform the following tasks in particular:

- Advising and informing
- Organising and coordinating the move into the care facility
- Support during the familiarisation period
- Handing over information to the care team

Concept of care transition

A continuously updated, harmonised concept was developed during the funding period in cooperation with the care transition teams and providers. An evaluation was carried out by the Catholic Foundation University in Munich.

25 years of the voluntary care transition support programme

The City of Munich has financed a voluntary funding programme for care transition in Munich care facilities for 25 years until 2024.

In 2023, the Munich City Council decided to end the voluntary care transition programme on 31 December 2024. From 2025, the funds released from this programme will be used to finance school social work in vocational schools for nursing and to increase the voluntary services of the "in-house day care" programme.
