  • Identity card or passport
  • Pension statement or pension certificate of the surviving dependant
  • Pension statement or pension certificate of the deceased
  • Last pension adjustment notification from both
  • Further proof of income (including salary, earned income, company pension)
  • If you are unable to come yourself, a person authorised in writing can receive the requested information or documents.
